Ashwagandha (Withania somnifera) has centuries of use in Ayurvedic tradition, and in the past fifteen years it has become the most extensively clinically studied adaptogenic herb in modern research — with a genuinely deep base of randomized controlled trials behind its effect on stress and cortisol specifically.
The clinical evidence, independently confirmed
The foundational trial is a 2012 randomized, double-blind, placebo-controlled study published in the Indian Journal of Psychological Medicine, in which participants taking standardized ashwagandha root extract experienced significantly greater reductions in serum cortisol than the placebo group, alongside improvements on standard stress and anxiety scales measured in the same participants.1 What makes ashwagandha’s evidence base unusually credible is that this signal has since been confirmed across multiple, independently conducted meta-analyses — not just studies funded by a single manufacturer:
- Marchi et al., 2025 (BJPsych Open), a meta-analysis of 14 RCTs and 713 participants that explicitly states it received no specific funding from any commercial or non-profit source, found a significant improvement in anxiety (SMD −1.62).2
- Bachour et al., 2025 (BJPsych Open), pooling 15 RCTs and 873 participants, found a significant reduction in serum cortisol.3
- Arumugam et al., 2024 (Explore), covering 9 RCTs and 558 participants, found significant reductions in perceived stress, anxiety (HAM-A), and cortisol.4
- Akhgarjand et al., 2022, a meta-analysis of 12 RCTs and 1,002 participants, similarly found significant effects on both anxiety and stress measures.5
Independently conducted, non-industry-funded meta-analyses landing on the same conclusion is a meaningfully stronger form of evidence than a single positive trial — it’s one of the more replicated findings in the entire adaptogen category.
The mechanism: how ashwagandha is thought to work
Ashwagandha’s active compounds, called withanolides, are believed to modulate activity along the HPA (hypothalamic-pituitary-adrenal) axis — the body’s central stress-response system — while also influencing GABAergic signaling and reducing inflammatory signaling pathways activated under sustained stress.6 This gives the cortisol-lowering effect a plausible mechanistic basis rather than being an isolated statistical finding.
What the evidence honestly says about certainty and timing
In the interest of precision, it’s worth noting that while the direction and significance of these findings are consistent, some formal evidence-grading frameworks characterize the overall certainty as moderate rather than high, largely due to variability between individual trials in dose, extract type, and population studied.7 This is a common and expected feature of botanical research generally, not a reason to discount the consistent direction of the findings across multiple independent teams.
Timing matters too: ashwagandha is not fast-acting. Most clinical trials showing meaningful stress and cortisol effects measure outcomes at six to eight weeks or later of consistent daily use, with more pronounced effects reported at higher doses over longer periods — this is a compound that supports the body’s stress response over time, not an immediate, single-dose effect.8
Why extract standardization is the detail that matters most
Not all ashwagandha supplements are equivalent, and this is genuinely the most practically important thing to know when evaluating a product. The great majority of positive clinical trials — including all the meta-analyses above — used KSM-66, a standardized root-only extract (as opposed to root-and-leaf combinations) standardized to at least 5% withanolides.8,9 The root is also the plant part with the longest documented traditional use. Clinical research has used doses ranging from 250 to 600 mg per day of standardized extract, with 300 mg appearing in multiple positive trials and more pronounced effects reported at 600 mg after eight or more weeks.8,9
Safety profile
A randomized, placebo-controlled safety study using 600 mg/day of KSM-66 for eight weeks found no significant changes in hematological, hepatic, or thyroid parameters, and a 2025 prospective, multi-center observational study followed standardized ashwagandha root extract use over a full 12 months.9 At considerably higher doses — above roughly 1,250 mg/day — some tolerability research has noted gastrointestinal irritation as a possible effect, which is a useful data point for staying within the well-studied dose range rather than a reason for general concern at typical doses.10
The practical takeaway
Ashwagandha’s cortisol and stress-reduction evidence is about as strong as botanical supplement research gets: a clear mechanistic rationale, a large and consistent body of randomized trial data, and — unusually for this category — confirmation across meta-analyses with no industry funding behind them. The two details that separate a well-supported ashwagandha product from a generic one are extract standardization (root-only, ≥5% withanolides, ideally KSM-66 or an equivalent standardized extract) and realistic expectations about timing — this is a compound that works over weeks, not hours.
